After detailing this information, claimants are required to provide a disability onset date. This is the date that you had to stop working due to disability and/or you were diagnosed with the disability. Use the accompanying medical records to pinpoint your approximate onset date.
Medicines And Medical Treatments
This section requires you to list all medical professionals, facilities and treatments you have received since you were diagnosed with a disability. You must include the contact information for the doctors and the addresses of the facilities. The SSA will contact the facilities to request your medical records.
They will also ask for the names of the prescriptions you take, the reasons you take them and the doctor that prescribed them. This section is lengthy and will require you to do a bit of research. Remember there is no time limit or application deadline for the ADR.
